Where to Claim Your Prize
Prizes exceeding $601 can be claimed at the Oklahoma Lottery Headquarters, any Authorized Lottery Claim Centers or by mail. Prizes of $5,001 or more must be claimed through Lottery Headquarters. Claim forms are supplied by any Lottery retailer or you can download a claim form below.
Documents You Need
For prizes of $601 and above, winners must fill out a claim form and present two forms of valid identification. The OLC must be able to verify the winner's identity and Social Security Number (SSN), therefore; at least one form of identification must specifically identify the SSN. The preferred forms of acceptable identification are a valid driver's license with current address and a Social Security card.

Agreement to Share Ownership and Proceeds of Lottery Ticket
The Agreement to Share Ownership and Proceeds of Lottery Ticket allows individuals the opportunity to share the proceeds of a winning ticket. One individual/representative will need to be designated as the individual representing the group. The group representative will need to complete Claim Form #1 and the top portion of the Agreement to Share Ownership Claim Form #2.
All parties sharing the proceeds will need to fill out areas 1 through 11 on the Claim Form #2. All parties will need to provide two legible forms of identification, including a copy of a photo id. If the entire group is not present at the time the claim is filed, the designated group representative will need to provide all of the above mentioned forms and identification information for each individual winner. Applicable tax will be deducted from each share. Unless written authorization is given to group representative, the check will be mailed.
